Grant Application Process Submit An Application

General Reminders:
If you have completed the preliminary steps and advance consultation, you are ready to begin preparing your application for submission. If you have questions about your application, the HCD Guidelines and the PND Guidelines documents are your best reference. If these documents do not answer your questions, you should contact the Trust at 336-397-5500.

Application Format:
The Trust has developed two different applications - one for capital projects and one for operating programs. See Page 10 of the HCD Guidelines or PND Guidelines if you need help determining which application best fits your program or project.

Instructions for completing each component of the application are included with the links below.
All completed applications must contain the following:

Application Deadlines

Health Care Division: March 15 and September 15
Poor and Needy Division: January 15 and July 15

Applications cannot be submitted online. They should be printed, signed, and mailed or hand delivered to the Trust office, at the following address:
