Step 1: Determine Eligibility and Fit

Determine if your organization is eligible for funding by the Trust.

Partners the Trust will fund:   Organizations the Trust will not fund:
  • Nonprofit 501(c)(3) organizations
  • Governmental entities
 
  • Individuals
  • Faith-based organizations without 501(c)(3)
  • Type III supporting organizations or
  • Organizations providing pass-through funds to an ineligible organization

 

Determine whether to apply to the Health Care Division or the Poor and Needy Division.

Both of the Trust's Divisions Both of the Trust’s Divisions invest in solutions to improve the quality of health for financially needy residents of their geographical areas. Please answer the following question to determine which Division guidelines to use for your application.

  • Is your organization located in Forsyth County? 

If you are located in Forsyth County, please review the Poor and Needy Division funding interests to determine if your proposal fits one of these program areas:

Increasing Self Reliance
Providing Basic Needs
 

If your organization is located in any other North Carolina county, please review the Health Care Division funding interests to determine if your proposal fits one of the four listed below.

Access to Primary Medical Care
Community-Centered Prevention
Diabetes
Mental Health and Substance Abuse
