Step 1: Determine Eligibility and Fit

Determine if your organization is eligible for funding by the Trust.

• Eligible organizations must be one of the following:
  • Qualified for 501(c)(3) exemption under the Internal Revenue Code and not a private foundation
  • Type III supporting organization
  • Governmental entity

• Grants are not made to:
  • Individuals
  • An organization to be used as pass-through funds for an ineligible organization
  • Faith-based organizations without a 501(c)(3) exemption

Determine whether to apply to the Health Care Division or the Poor and Needy Division.

• Health Care Division
  • Responds to health and wellness needs - treatment and prevention
  • Invests in solutions to improve the quality of health for financially needy residents of North Carolina

• Poor and Needy Division
  • Responds to basic life needs - basic needs and self-reliance
  • Invests in solutions to improve the quality of health ONLY for financially needy residents of Forsyth County
